as an Age-Friendly Health System

LOWVILLE, NY Lewis County
General Hospital (LCGH) has achieved special recognition as an
Age-Friendly
Health System Committed to Care Excellence in our ongoing efforts to
ensure age-friendly care for all older adults we serve.
We are proud to
receive this recognition, said Marcy Teal, LCGH Chief Nursing Officer.
The Age-Friendly Health Systems initiative supports our ongoing goal to
provide the best care for older adults.
Over the last year,
LCGH has collected data on the number of older adults who received a set of
evidence-based elements of high-quality care, called the 4Ms. These include: What Matters (ensuring care aligns with each
person's preferences), Medication (making sure the medications used do not
interfere with overall age-friendly care), Mentation (addressing mental
health issues such as dementia and depression), and Mobility (supporting
older adults in maintaining movement and function). We are excited to continue
our commitment to the reliable practice of the 4Ms.
The interdisciplinary team (pharmacy,
social work, case management, physical therapy, quality, and nursing) helped
lead the way in achieving both levels of this Age-Friendly recognition,
says Teal. It reflects the vital role the care team has in supporting
older adults with high-quality, person-centered care and ensuring dignity,
safety, and compassion at every step.
More than 5,200
hospitals, clinics, nursing homes, and home health care organizations have
achieved the Age-Friendly Health Systems Committed to Care Excellence
recognition. Lewis County General Hospital is proud to join these
organizations in putting the 4Ms into practice, which has reached more than
5,470,000 older adults globally.

Burnett
