Input Wanted from Lewis County Residents for Health Improvement Survey

 

Lewis County Public Health (LCPH) wants to know what Lewis County residents think about health in our community as we develop our next health improvement plan. For this reason, we have released a community health survey that is open to every Lewis County resident aged 18 and older. The survey can be accessed at surveymonkey.com/r/LCHIP2022 or by scanning the QR code located to the right and at the bottom of the page. The survey link will also be shared on lewiscountypublichealth.com and our Facebook page.

 

LCPH partners with the North Country Health Compass Partners and Fort Drum Regional Health Planning Organization (FDRHPO) to routinely complete an adult community health survey (via phone) that is used to inform our Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). The CHIP is a 3-year plan that is then implemented by LCPH staff with the support of community partners. Priorities addressed in previous plans include “Prevent Chronic Disease”, “Promote Healthy Women, Infants, and Children” and “Promote Well-Being and Prevent Mental and Substance Use Disorders”. The hope of this health improvement survey is that it will complement the results of the adult community health survey and provide additional insight into our community’s unmet health needs.

 

The survey addresses topics including community health, access to care, food security & healthy eating, physical activity, mental & emotional health, chronic conditions, disability, caregiving, and COVID-19. The survey takes about 7-10 minutes, and can be completed on any mobile device, desktop, or laptop computer, or call 315-376-5453 during business hours, Monday – Friday, 8:30 am – 4:30 pm and staff will help callers complete the survey. Community members can also stop by the LCPH booth at the Lewis County Fair, Tuesday, July 19 – Friday, July 22 from 11 am – 7 pm to complete the survey with the assistance of LCPH staff.

 

LCPH is committed to promoting and protecting the health of our community. While multiple data sources will be considered during our CHA/CHIP planning process, the feedback we receive in this survey will be invaluable. We would like to thank Lewis County residents in advance for responding to the survey and providing us with their honest feedback. We look forward to reviewing the results, sharing the results, and then using the results to better serve our

community.
