Receiving COVID-19 Vaccine Outside of New York State to Report Status to
Their Office

Vaccinated Outside of New York State?
Here is How to Have Your Vaccine Count!
Were you
vaccinated outside of New York State? Do you want your vaccination to count
toward the 70%
goal for Lewis
County? Do you want an Excelsior Pass? Lewis County Public Health can help!
Lewis County
Public Health is encouraging any resident that received their COVID-19
vaccine outside of
New York state to
report their vaccine to our office. Residents will need to call the Public
Health office to
provide the
following information: name, date of birth, address, phone number, and
proof of vaccination
from the other
state. Proof of vaccination can be a picture of your vaccine card or
vaccine record, this
proof can be
emailed to us or brought into our office.
Public Health
will enter this information into the New York State Immunization Registry.
This will allow
residents who
wish to obtain an Excelsior Pass to do so. Excelsior Pass is a fast, free,
voluntary, and
secure way to
present digital proof of COVID-19 vaccination. If residents choose to
participate, they can
download the
Excelsior Pass Wallet app from the Apple App Store or Google Play Store and
follow the
instructions to
retrieve your Pass for being fully vaccinated. Alternatively, residents may
retrieve their
Pass from the
Excelsior Pass website at https://epass.ny.gov. From the website, they can
print a paper
Pass, take a
screenshot of your Pass, or save it to the Excelsior Pass Wallet mobile
app.

Public Health appreciates all the steps Lewis County Residents have taken, including
getting the
COVID-19 vaccine, to slow the spread of the virus. Anyone who wishes to
report their out of
state vaccine or
who is looking for more information about the COVID-19 vaccine can call our
